Building an intake source log for offshore legal support
Keep each intake fact tied to its source, owner, and review state before a law firm decides what happens next.
Identify the source
Record whether each fact came from a call, approved form, email, or existing matter record. Keep the original wording available and mark the time it was captured.
Separate facts from flags
A support worker can highlight missing names, dates, entities, or attachments. The firm decides whether a fact changes eligibility, conflicts, urgency, or advice.
Make review explicit
Give every row a reviewer and state: received, unclear, awaiting firm input, or released. This prevents an incomplete intake from looking final.
