Turn repeated corrections into usable process evidence without blaming workers or hiding attorney judgment calls inside a quality score.
Use a short approved vocabulary
Choose plain codes that match the work, such as missing source, wrong field, format mismatch, duplicate item, unclear instruction, or reviewer decision. Keep the original example with each code.
Separate error from changed instruction
A correction caused by a new firm instruction is different from a missed field or unsupported assumption. Record both the source version and the reason the output changed.
Turn patterns into owners
Review recurring codes with the manager and process owner. They decide whether to improve the example, adjust the queue, coach the worker, or change the review gate.
