Name the missing evidence
For each gap, record the matter, record type, expected location, search performed, and date checked. A missing result is useful only when the search boundary is clear.
Preserve the uncertainty
Do not mark a record destroyed, unavailable, or outside policy without an authorized source. Keep conflicting system results and duplicate identifiers visible.
Route the decision
Send the gap queue to the firm’s records owner with source links and proposed next questions. The firm decides retention, hold, recovery, or closure.
