Medical-chronology indexing support for law firms
Arrange treatment records by provider and date while qualified reviewers control medical interpretation, causation, and case conclusions.
Use source dates and providers
Index each record by provider, service date, document date, record type, patient identifier used by the firm, and source file. Keep records with multiple dates explicit rather than choosing one silently.
Show gaps and duplicates
Mark missing periods, repeated pages, conflicting identifiers, and unreadable notes as exceptions. Do not infer treatment, diagnosis, recovery, or causation from an administrative sequence.
Deliver a reviewable chronology
Sort the approved records, link entries to their sources, and include a gap list. The attorney, medical reviewer, or expert decides what the chronology means and what additional records are needed.
